Care and Support

 

We offer individual, mutually agreed, person centred care and support to people with the aim of encouraging individuals to become as independent as possible. We listen to our service users, and then work with them to provide the support they need to take control of their lives and reach their goals.

 

Support includes assistance with personal care, emotional support, money management, health, development of skills, accessing services, maintaining friendships, participating in employment and educational opportunities and support to move on

Outline of Services.

In the last year Prospect has provided services to over 500 people living in Surrey, the London Borough of Croydon and the London Borough of Sutton. We currently operate the following services:

Seven residential care homes in South East Surrey - providing personal care to 53 people with a learning disability.

A care home in South East Surrey providing 5 beds for breaks for carers of young people (18-30 years) with a learning disability.

Thirty independent flats in East Surrey with visiting support.

A Supported Living Service in Surrey and Croydon which provides support to 43 people on an individual basis living in independent housing and 37 tenants in 9 Group HOmes. The service provides support to people with a learning disability, acquired brain injury, sensory impairment, and people with Asperger's Syndrome. Support ranges from 1 hour a week to 24 hours a day.

A Supported Living Service for people with mental health problems providing support to 34 service users. This provides support to 6 people on an individual basis living in independent housing and 28 tenants in 4 group homes.

A Supported Living service in West London which provides support to 5 people on an individual basis living in independent housing.

A Carers Support Service in the London Borough of Sutton providing care staff and volunteers to 138 carers to enable them to take short breaks from their caring responsibilities.

A Carers Support Service in South East Surrey providing domiciliary support to over 40 young people (16-30 years) with a learning disability to allow their carers to have a break.

A Tenancy Support Service in East Surrey providing tenancy support to up to 106 people at risk of immediate homelessness. This is provided in partnership with Raven Housing Trust.
